Bring more interactivity to your courses by adding custom assessments and feedback forms directly to your store items. Whether you want to check in with clients after a module, gather feedback, or add a completion form to a teacher training program, you can build it with custom forms and embed it right where your clients need it.
Step 1: Create Your Custom Form
Before adding an assessment to a store item, build the custom form you'll use:
- Create a new custom form and give it a clear name (for example, "200hr Yoga Teacher Training Completion Form").
- Add the fields you want, choosing from text area, number, multiple choice, multiple select, date, and more.
- Mark any fields as required if you want to ensure clients complete them.
- Map each field to a custom field, or create a new custom field if one doesn't already exist.
- Add a confirmation message to display after submission, or set up a redirect to send clients to a different page.
Once your form is ready, you're all set to add it to a store item.
Step 2: Add the Assessment to a Store Item
- Head to the store item you'd like to add the assessment to (for example, a Yoga Teacher Training program).
- Open the module or content area where you want the assessment to live. You have two options for placement:
- Same page as the content — add the assessment to an existing content block within the module.
- Separate page — add a new content section so the assessment appears on its own page. Here, choose the content type you'd like to use (text is a great option for this).
- Choose a style for your form.
- Click to edit, then select your custom form you have previously created(for example, your teacher training completion form).
- Adjust the color settings so the form blends with the rest of your site.
Once saved, all the questions from your custom form will display, ready for clients to complete and submit.
Previewing Your Assessment
Before publishing, preview your store item to see exactly what your clients will experience. Depending on how you've set things up, you might see:
- The module and assessment as two separate tabs, or
- The assessment appearing at the bottom of the same page as the content.
Both approaches work well — it just depends on the experience you want to create. If you'd like clients to be able to refer back to the course material while completing the assessment, keeping them on the same page is a great choice. If you'd prefer a distinct, standalone assessment experience, a separate page works beautifully too.
Note: These assessments are designed for feedback and engagement rather than grading. They don't calculate scores, percentages, or correct/incorrect answers — think of them as a way to check in with your clients and make your course feel more interactive.
